Come join an amazing team that focuses on elevating the human connection!Please call(516) 367-0193 to schedule an appointment to meet with a hiring manager today!We are located at:900 Woodbury Road, Woodbury NY 11797Responsibilities for the LPN include:Direct Supervision--- Closely mentors, guides and assists the Lead Resident Care Assistants in all aspects of their jobResident CareRecognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulationsAssists with coordination of resident care needs with community providersMeasures and accurately records vital signs (temperatures, pulse, respiration, blood pressure weight and height) where appropriate as per state regulationsRegularly monitors each resident's medication and treatment profile to ensure each medication and treatment is administered as ordered and documented accuratelyAccurately assesses, communicates, and documents residents' status and ensures proper follow through and documentationObserves and reports any significant changes in resident behavior and health to the Resident Care Director, physician, family, Resident Care Associate, and Executive Director per state regulationsSuccessfully implements the plan of careConducts regular wellness visitsRequirements:--- Graduate of an approved LPN / Licensed Practical Nurse program (per state requirements)--- Current state licensed as an LPN / Licensed Practical Nurse (as required by state regulation) and CPR certification--- Previous experience working with the elderly in a paid or volunteer position highly desirableBENEFITSWe believe in offering our employees competitive payHourly wage up to $36.00 based on experienceFull-time and Part-time Positions available for the 3pm - 11pm shifts
