different specialties. Job Description Responsible for coordinating
the individualized care of patients throughout the continuum of
care services, functioning as patient advocate, educator, key point
of contact and liaison between the patient and all other
appropriate services. Navigate the VT patient population through
the pre-procedure testing process by providing education and
guidance. Use foundational cardiology knowledge and experience to
gain extensive knowledge of electrophysiology process and
procedures. Function as collaborating member of the health care
team, but also must be able to act independently to assess patient
and physician needs to ensure appropriate hand-offs so that
patients receive services promptly and efficiently. Maintain a
national registry database for VT procedures in collaboration with
the quality team. Ensure all requirements are met at expected
timelines prior to submission. Provide pre/post, disease process
education to patients via phone and/or in-person on the day of
testing for any of our VT procedures. Review educational materials
with patient and/or family. Return calls to patients to discuss
concerns, questions, and provide guidance. Qualifications Bachelor
of Science in Nursing Five years’ experience as a nurse CT RN
License or PA/APRN License Two years supervisory/management and
patient/staff education experience required We take great care of
