reimbursement What You’ll Need NYS RN license required Associate’s
Degree in Nursing (experienced candidates) or BSN (new graduates) 1
year of RN experience in managed care, home care, acute care, or
nursing home preferred Certified Case Manager (CCM) preferred
Bilingual proficiency required What You’ll Do Provide case
management for members, addressing medical, social, psychological,
physical, and spiritual needs Develop, implement, and monitor care
plans in collaboration with PCPs, caregivers, and the care team
Ensure high-quality, personalized care for all members Support the
goals and values of the managed care health plans
