MASTER SOCIAL WORKER (DISCHARGE PLANNING) Position Type: Temporary,
Maternity Leave Coverage, to start ASAP through August Location:
On-site at Client Hospital Payrate: $35-42/hr DOE Shift: 8am-4:30pm
with rotating weekend coverage Role Overview: Our agency is seeking
a compassionate and organized Licensed Master Social Worker (LMSW)
for a temporary assignment providing discharge planning services at
our client's hospital facility. In this vital role, you'll assist
our client in ensuring patients transition safely and successfully
from hospital to home or appropriate care settings. Key
Responsibilities: Conduct comprehensive discharge assessments to
identify patient needs and barriers Develop personalized discharge
plans that address medical, psychological, and social factors
Collaborate with the client's multidisciplinary healthcare teams to
coordinate seamless transitions Connect patients with community
resources, support services, and follow-up care Facilitate family
conferences to ensure understanding of care plans Complete timely
documentation according to hospital and regulatory standards
Navigate insurance approvals and authorization processes Advocate
for patients' needs throughout the discharge planning process
Qualifications: Current LMSW license required Experience in
hospital discharge planning preferred Strong knowledge of community
resources and social service systems Excellent communication and
interpersonal skills Ability to work effectively in fast-paced
healthcare environments Problem-solving skills with a
patient-centered approach Schedule Note: This position includes
weekend coverage to ensure continuous patient care. Make a
