Job Description Job Description About the Role: The Family Nurse
Practitioner (FNP) plays a crucial role in delivering comprehensive
healthcare services to patients of all ages within the community.
This position involves assessing, diagnosing, and managing a wide
range of health conditions, ensuring that patients receive
high-quality care tailored to their individual needs. The FNP will
also focus on patient education, empowering individuals to make
informed decisions about their health and wellness. Collaboration
with other healthcare professionals is essential to provide
holistic care and improve patient outcomes. Ultimately, the FNP is
dedicated to promoting health, preventing illness, and managing
chronic conditions, contributing significantly to the overall
well-being of the community. Schedule - Rotating Weekends (Saturday
and Sunday 8a-1p) in Toms River and Brick Minimum Qualifications:
Master's degree in Nursing with a specialization as a Family Nurse
Practitioner. Current and valid Nurse Practitioner license in State
of New Jersey Certification as a Family Nurse Practitioner from a
recognized certifying body. Preferred Qualifications: Experience in
outpatient setting Minor suturing required Responsibilities:
Conduct thorough patient assessments, including physical
examinations and health histories. Diagnose and manage acute and
chronic health conditions, developing appropriate treatment plans.
Provide patient education on disease prevention, health promotion,
and medication management. Collaborate with interdisciplinary teams
to ensure comprehensive patient care and continuity of services.
Document patient interactions and treatment plans accurately in
electronic health records.
